REFERRING PROVIDERS

OUR SERVICES

The Regional Center for Infectious Disease gladly accepts new patient referrals from providers.

We most commonly treat:

 

  • Pneumonia

  • Meningitis

  • HIV/AIDS

  • Sexually-transmitted Infections

  • Hepatitis 

  • Hospital acquired Infections

  • MRSA

  • c. difficile Infections

  • Skin Infections

  • Urinary Tract Infections

  • Malaria

  • Tuberculosis

  • Fever of Unknown Origin

  • Endocarditis

COVERAGE

All insurance co-payments are required at time of check-in.

Once we receive pertinent records, demographics and the referral form, we will contact the patient to schedule an appointment. We review all new patient referrals to ensure that we treat the particular diagnosis that is being referred.

MEDICAL RECORD RELEASE

In the event you need copies of your medical records you will need to complete a Release of Information form that has to be signed in our office. It can take up to 14 days to complete and the records will be mailed to your home
