PHARMACY VISIT

RCID provides access to a pharmacy team comprised of two HIV/Infectious Disease pharmacists and one patient advocate embedded within the clinic. This pharmacy team has an important role in caring for our patients with HIV and some of their responsibilities include:

 

  • Reviewing patient’s charts to ensure optimal pharmacotherapy

  • Providing one-on-one patient education

  • Consulting with patients and medical providers regarding medication-related problems

  • Drug interactions

  • Adherence issues

  • Immunizations

  • Side effect management

  • Interpreting resistance test results

  • Initiating and managing ART

  • Ordering lab tests

  • Initiating, adjusting, or discontinuing medications for concomitant disease states

  • Medication access/acquisition

  • Benefits investigation, co-pay and manufacturer assistance

  • Transitions of care

Our pharmacist team will see a wide variety of patients at the RCID including HIV infected patients, Hepatitis C, Hepatitis B, PrEP, PEP, and other infectious diseases.

 

When coming for your pharmacy appointment, please remember to bring:

 

  • All of the mediations you are currently taking or a list of current medications

  • Insurance information

  • Any other paperwork deemed necessary by the clinic

  • If you are uninsured (Hepatitis C, HIV, PrEP, etc):

    • Proof of income (last 2 paysubs or W2 Form)

    • Proof of residency (driver’s license with current address or bill statement)
